Refusal of care formWhen to use:
- When a patient or their caretaker refuses any form of clinical care as recommended by the doctor
Definitions:
- Adult (anyone over the age of 18)
- Minor (anyone less than the age of age of 18)
- Impaired decisional capacity (an adult, as defined above, with impaired decisional capacity means that at the time a decision needs to be made he or she is unable, temporarily or permanently and irrespective of the cause – to make the decision for him- or herself on the matter in question; or to communicate his or her decision on that matter)
Instruction of use:
- Ask the clinic nurse to download the form from the knowledge base
- Ask the sister to ask for a witness if the patient or person refusing care is alone
- The nurse fills in the form as required
- The nurse, patient or the person refusing care and a witness should all sign the form
- The nurse should take a pic and upload it to the patient's record
- The doctor confirms upload and checks all signatures on the form
