You can write a referral letter for the patient to take along to the specialist as needed. Print a copy of the referral letter and report for each specialist. You can also email copies if the patient has the specialist details.

A referral to visit an audiologist is made for patients with an At risk hearing result. Additionally, if the patient indicates that he/she suspects that they might have a hearing problem, the patient needs to be referred to an audiologist and this will be indicated on the VHC report.
The hearing test will determine the patient’s speech reception threshold (SRT). The hearing risk is calculated based on the SRT values. There are four risk categories:
1. No risk (hearing is good);
2. Low risk (hearing is OK),
3. Moderate risk (hearing is OK), and
4. At risk (hearing problem).
A referral to visit an optometrist is made for patients with an At risk vision result. If the patient indicates that he/she suspects that they might have a vision problem, the patient needs to be referred to an optometrist and this will be indicated on the VHC report.
The vision test will determine the visual acuity as a LogMar and Snellen value for both the left and right eye. The vision test result will indicate a ‘No Risk’ implying the patient’s vision (tested with or without glasses) is good or ‘At Risk’ which implies the patient should be referred to an optometrist.
